Living safely with anosmia.

Smell is a warning sense. When it stops working, a few everyday risks that other people handle without thinking become things you have to handle on purpose. Most of the answer is equipment you buy once and habits you set up once. Gas, fire, food, cooking, cleaning products, and the conversations worth having at home and at work. Educational information, not medical advice.

What the risk actually looks like

The NIDCD, part of the National Institutes of Health, puts it plainly. Your sense of smell often serves as a first warning signal, alerting you to the smoke of a fire, spoiled food, or the odor of a natural gas leak or dangerous fumes.

One study is cited more than any other for the size of that gap. Among 445 patients at a university smell and taste clinic, 37 percent of those with olfactory impairment reported at least one hazardous event, against 19 percent of those with normal olfaction. For patients with anosmia it was 45.2 percent. Cooking accounted for 45 percent of those events, eating spoiled food or a toxic substance 25 percent, and undetected gas leaks 23 percent.

A clinic population is not the general population, so read those percentages as a map of where to spend attention rather than a forecast. The useful part is that the events cluster, so a short list of fixes covers most of them. Our Resources page has the checklist version of what follows.

Gas detectors, if you have gas at all

Natural gas is odorless. So is propane. The smell people associate with both is an odorant added on purpose so that a leak announces itself, and that system was designed around a working sense of smell. If yours is not working, it does nothing for you. A detector puts the warning back.

Mounting height depends on which gas you have

Methane, the main component of natural gas, has a vapor density of 0.55 against air at 1, so it rises and gathers high in a room. Propane runs the other way at 1.6, and the same fact sheets note that it may pool or settle in low areas and stay there a long time.

Natural gas: mount the alarm high, near the ceiling, within a few feet of the appliance. Propane: mount it low, close to the floor. Propane is common in rural homes, RVs and boats, and an alarm on the wrong wall can sit through an entire leak. Follow the manufacturer’s instructions for exact height, since they are the ones who tested it.

Buy one that has been tested

In 2026 the Consumer Product Safety Commission told consumers to stop using a 4 in 1 plug in gas and carbon monoxide detector sold under 18 brand names online, after 91 reports of it failing to alarm during actual leaks. CPSC advice is to only buy carbon monoxide detectors meeting the UL 2034 safety standard. For household gas alarms the standard is UL 1484, Residential Gas Detectors. Look for a listing mark from a recognized testing laboratory.

What to do when it goes off

Agree this in advance with everyone in the house. The federal pipeline safety agency’s guidance for a suspected leak is to make sure gas appliances are turned all the way off, then leave the area. On the way out, do not start an engine of any kind, strike a match, use a telephone or cell phone, or turn any electrical switch on or off. Any of those can ignite gas in the air. Once well clear, telephone 911 from a neighbor’s house and warn others against entering if it is safe to do so.

Smoke and carbon monoxide alarms

Everyone needs these. You need them to be right, because the warning most people get arrives before any alarm does, as the smell of something burning. Wiring behind a wall, a motor overheating, a dish towel against a hot burner. You will find out when the alarm sounds.

Smoke alarms

The U.S. Fire Administration’s guidance is to put smoke alarms inside and outside each bedroom and sleeping area, and on every level of the home, on the ceiling or high on the wall. Where you have more than one, choose interconnected alarms so that when one sounds, they all sound. That upgrade does more for an anosmic household than anything else here, because it removes the assumption that somebody is near enough to notice.

Test monthly. Replace a 9 volt battery, or the backup battery in a hardwired unit, at least once a year, and replace the whole alarm every 10 years. Sealed 10 year lithium units take the annual change off your list, and the USFA says to follow the manufacturer’s instructions on replacing those.

Carbon monoxide alarms

Carbon monoxide is odorless to everybody, so on the gas itself you are no worse off than anyone. What you lose is the evidence around it: exhaust drifting in from an attached garage, a generator too close to a window, a furnace burning badly.

The CPSC advises installing battery operated CO alarms, or CO alarms with battery backup, on every level of the home and outside sleeping areas, interconnected where possible, with batteries checked regularly. If one activates, get outside to fresh air immediately, then call 911. The symptoms to know are headache, dizziness, weakness, nausea, vomiting, sleepiness and confusion.

Food, when the sniff test is gone

Nearly all consumer food safety advice quietly assumes a working nose. Even the official version does: the USDA tells consumers to evaluate the quality of a product before eating it, which in practice means opening the container and smelling. Since that is closed to you, replace it with time, temperature and labels.

Dates help less than people assume. The USDA is clear that Best if Used By indicates best flavor or quality and is not a purchase or safety date, that Sell By tells the store how long to display the product, and that Use By marks peak quality rather than safety outside of infant formula. None of them are required by federal law. Use them as a starting point.

Run on time and temperature

Keep the refrigerator at 40°F (4°C) or below, the freezer at 0°F (-18°C) or below. Buy a cheap fridge thermometer and look at it, because a dial marked 1 to 5 tells you nothing.
Refrigerate leftovers within 2 hours of cooking, or 1 hour if the room is above 90°F.
Leftovers keep 3 to 4 days refrigerated, or 3 to 4 months frozen.
Date mark everything as it goes in. Masking tape and a marker on the lid, with the day it was cooked.
Cook to temperature, not appearance. Poultry 165°F, ground meat 160°F, beef and pork steaks and roasts 145°F with a 3 minute rest, fish 145°F, egg dishes 160°F.

What your eyes can still catch

Sight and touch do not replace smell, but they pick up a fair share. Mold of any color. Slime on deli meat or chicken. Discoloration. A bulging can or jar lid, or a vacuum pack that has puffed up, which means something inside is producing gas and it goes out without further inspection. Milk is the one people miss most, since the old test was purely a sniff. Buy smaller cartons so it turns over faster, and pour a little into a clear glass first, where lumps and a yellow cast show up.

When something is borderline, throw it out or ask someone to check it. A container of leftovers costs less than two days of food poisoning.

Cooking

Cooking was the largest category in the Santos study at 45 percent of hazardous events, and the one with the most straightforward fixes.

Look at the flame, every time. A gas burner that clicks without catching keeps releasing gas into the room, and the odorant will not reach you. Glance at the burner after you turn it on, and at the whole hob before you leave the kitchen.
Weigh induction or electric when the range needs replacing. It removes the unlit burner problem completely.
Timers, not attention. Most people catch food burning by smell minutes before there is smoke. Set a timer for anything in the oven, and a labelled one on your phone if you leave the room.
Run the extractor or open a window. A kitchen filling with smoke is obvious to everyone except you.
Never leave oil heating unattended. The smell of oil approaching its smoke point is the usual early warning, and it is one you will not get.

Cleaning products, and the rest of the house

Never mix cleaners

The CDC’s wording is blunt. Never mix household bleach, or any disinfectant, with any other cleaner or disinfectant, because it can release vapors that may be very dangerous to breathe in. Bleach with an ammonia based cleaner is the classic kitchen accident.

Chlorine gas is one of the things that reaction produces. For most people its pungent, irritating smell is what drives them out of the room in the first seconds. NIOSH puts chlorine at immediately dangerous to life or health at 10 parts per million, with burning eyes, coughing, choking and chest pain among the effects. If that warning never reaches you, the remaining protection is not creating the gas. Use one product at a time, rinse between products, and open a window while you work.

Personal care on a schedule

Most people monitor themselves by smell all day and never notice they are doing it. Without that feedback, stop relying on perception and run on a schedule instead. Wash clothes on a fixed rotation whether or not they look dirty, shower and change on a set routine, and treat deodorant as a step in getting dressed rather than a decision.

Then ask one person you trust to tell you straight if something is off. Nobody volunteers this unprompted, and being asked once, plainly, makes it far easier for them to say it later.

One last thing about the building itself. Hot electrical insulation and an overheating appliance both give off a smell well before anything is visible, so check cords and outlets now and then for scorch marks or a plug that feels hot, keep extension cords out from under rugs, and keep space heaters clear of fabric.

The people around you

Children in an anosmic household

Children take this on easily when it is framed as ordinary rather than frightening. The version that works is short. If you smell smoke, gas or something burning, tell me straight away, even if you think it is nothing. Practice the alarm sound so it is familiar, and walk through where everyone goes if it sounds at night. Keep it as one household rule among several rather than a job a child carries. The alarms are the safety system; a child is a useful extra pair of senses.

Households also do better with an agreed convention, because otherwise speaking up feels rude. Make it a rule that anybody can say that milk smells off, or something is burning, or you should change that shirt, and nobody questions it or takes offense. Once it is a rule it stops being a comment on you, and that one agreement replaces a dozen awkward moments a year.

Work

Some jobs use smell as a safety sense: kitchens and food handling, healthcare, laboratory and chemical work, firefighting, gas fitting, and industrial settings where a leak announces itself by smell. If yours is on that list, quietly working around it is the worst option, because the gap stays in place with nobody accounting for it.

When to get medical input

Everything above assumes smell loss you already know about. If the loss is new, or nobody has looked at it, that is a separate errand worth doing. The NIDCD’s advice is to talk with your doctor if you are experiencing a smell disorder, partly because diagnosis matters in its own right: a smell disorder can be an early sign of Parkinson’s disease, Alzheimer’s disease or multiple sclerosis. The specialist is an otolaryngologist, usually called an ENT.
